Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to getting struck and run over by a refuse collection vehicle: (a)Various locations, including, but not limited to, Lancaster County Prison, 625 East King Street, Lancaster, Pennsylvania - Employees rode in the cab of the Crane Carrier rear-loader garbage truck, license plate number YPM6798, vehicle identification number S1CYCCG488RT040768, with the passenger door open when entering the prison to collect trash. As the driver pulled the truck up and to the right in the work release yard, the helper (seated in the right seat of the cab) dismounted the moving truck, exposing this employee to getting struck and run over by the moving vehicle, as happened on October 12, 2006, when the right front wheel of the truck, situated behind the cab of the vehicle, ran over the helper. ABATEMENT NOTE: Among other methods, one feasible and acceptable abatement method to correct these hazards includes complying with the American National Standard for Equipment and Technology and Operations for Wastes and Recyclable Materials - Mobile Wastes and Recyclable Materials Collection, Transportation, and Compaction Equipment Safety Requirements (ANSI Z245.1-1999), specifically by: Monitoring employees and taking appropriate corrective action to ensure that employees comply with work practices, such keeping cab doors closed and latched during transit and remaining in driving or riding positions during collection activities until the vehicle has come to a complete stop and the brakes are set (Paragraph 6.2.3.2 of ANSI Z245.1- 1999) VERIFICATION REQUIRED
