Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to being struck and or crushed by an improperly shored overhead roof that was being demolished. One feasible and acceptable method to correct this hazard is to properly erect shoring towers according to procedures established by the manufacturer of the towers to ensure that their design limits are not exceeded: A) LOCATION: 12 MAIN ST., HOLYOKE, MA ON OR ABOUT 1/19/2006, THREE SHORING TOWERS THAT WERE ERECTED TO SUPPORT AND SECURE THE ROOF OF THE BUILDING, WERE NOT ERECTED ACCORDING TO THE MANUFACTURER'S DESIGN OF THE SYSTEMS. THE TOWERS WERE CRIBBED WITH MULTIPLE PIECES OF 3"X 12" WOOD IN AN ATTEMPT TO GAIN ADDITIONAL HEIGHT ON THE TOWERS. THE CRIBBING WAS LAID ACROSS THE TOP OF SHORING HEADS THAT WERE PART OF THE SYSTEM, AND NOT WITHIN THE HEADS THEMSELVES. THIS CREATED AN UNSTABLE SYSTEM AND REDUCED THE MAXIMUM LOAD CAPABILITIES OF THE TOWERS THAT WERE ESTABLISHED BY THE MANUFACTURER.
