Text For Citation: 01 Item/Group: 001 Hazard: GUARDING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recogn ized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to cause crushing injuries between the elevator and elevator shaft wall. a) Worksite located at 135 Monroe Street, Newark, NJ Employees operated a Newark Elevator Company freight elevator with th e car gate secured in the upright position and the gate interlock overridden by tying back the electrical box of the interlock contact switch. Violations observ ed on or about 4/12/2005. ONE FEASIBLE ACCEPTABLE ABATEMENT METHOD, AMONG OTHERS , TO CORRECT THIS HAZARD IS: to release the elevator car gate, remove the tie ba ck that holds the interlock switch and follow the guidelines set forth in ASME A 17.3-1996 Safety Code for Existing Elevators, Section 3.4.2 "Car Doors and Gates" and Sect ion 2.7.1 Hoistway Door or Gate Locking Device which states (a) Interlocks Requi red for Freight elevators.
