Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards at the elevator shafts: Location: 70 Washington St, Brooklyn, N.Y. On or about 11/03/04. a)Employees were exposed to fall hazards in the twelve story elevator shafts and exposed to being struck by the moving elevators because the elevator hoistway doors were not secured. Employees were opening the hoistway doors to look for the elevator and shout to the elevator operator. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS: Provide secured elevator hoistway doors as required by ANSI Standard A17.3- 2002 par. 2.7.1(b) which requires that elevator hoistway doors shall be equipped with hoistway door interlocks to prevent access to the elevator shaft. Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
