Text For Citation: 01 Item/Group: 001 Hazard: PUNCTURE
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to puncture wounds, lacerations, and impalement due to the protrusion of steel pins, metal brackets, supports and threaded rods: In the following instances employees were exposed to puncture, laceration, and impalement hazards: a)Hull 500, Engine Room 1, 1st plat, port side, frame 215 - 4, 3/4" threaded studs protruded 4" above the walking/working surface. b)Hull 478 - At numerous locations throughout the hull insulation pins protruded into the work space. c)Hull 478, State Room, 2 Tack, 338, 4L - 10, 1/2" threaded studs extended 6" above the walking/working surface. d)Hull 478, 01-338-3-Q - 4, 1" angle iron was protruding 17.5". e)Hull 478, 01-314-1-L, State Room - 16, 1/2" threaded rods protruded 4" above the walking/working surface. f)Hull 478, Berthing 1 - 2, 1" pieces of angle iron was protruding 4" into the walking/working area. g)PO2, Staging 6C, Hull 500, unit 313, 2nd plat, 1C gyro 11 - 13, 3/4" cable tray studs protruded 3" into the walking/working surface. h)Blast Building #3, Unit 313, Birthing Level (3rd), Starboard side (3/24/05) - Unprotected metal studs from floor. i)Blast Building #3, Unit 313, Birthing Level (3rd), Port side (3/24/05) - Unprotected angle iron (ladder supports) from floor. j)Assembly Building in front of large east double doors - The studs, approximately 4" high x 11/16" squared top, used to install wiring trays on the ceiling were inadequately guarded. k)Assembly Building Unit 232, AMR2 - The 6 protruding studs in the ~12 1/4" walkway, approximately 4" x 11/66" with squared top, used for cable tray mounting brackets on the ceiling were not protected. Among other methods one feasible and acceptable method of abatement is to install covers over the protrusions, which will reduce the likelihood of puncture wounds to the head, body and limbs.
