Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees. Employees,working in the machine room and in or around the elevator shaft, were exposed to struck by and fall hazards resulting from a cut wire rope that was secured in place by a single fist grip that had not been applied and tightened in accordance with manufacturer's specification. Elevator #19 - 1 Beacon Street, Boston, MA: On 4/15/05, employees were exposed to fatal injuries while working in the machine room and in or around the elevator shaft, resulting from a cut elevator wire rope that was secured by an single fist grip that was not tightened to the manufacturer's specifications. 1) The employer utilized a fist grip in a manner not recommended by the manufacturer. 2) The employer failed to train the employees on the manufacturer's specifications and limitations for use of fist grips. 3) The employer failed to ensure that a torque wrench was used by employees to determine if the bolts on the fist grip were tightened to the manufacturer's specifications. 4) The employer failed to ensure that employees performing re-roping work were provided with a comprehensive written Job Hazard Analysis for the work conducted on elevator car number 19. Methods to correct the condition, among other methods, are as follows. 1) Ensure that fist grips are used only in accordance with the manufacturer's specifications. 2) Train the employees involved with elevator re-roping operations on the manufacturer's specifications and limitations for use of fist grips in accordance with the manufacturer's specifications. 3) Utilize another method of securing the cut wire rope that is approved by the manufacturer for re-roping operations. 4) Ensure that management provides employees with a comprehensive written Job Hazard Analysis for re-roping operations.
