Text For Citation: 01 Item/Group: 001 Hazard: EXPLOSION
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm: Well site, Barnes 1-4, Texas County, Oklahoma: On or about September 12, 2005, the employer's training program was not appropriately documented to address safe assembly of a well perforation gun. This lack of training contributed to the detonation accident injuring two employees, one fatally. Among other methods, one feasible and acceptable method to correct would be to appropriately train employees on safe practices for detonation of oilfield explosives.
