Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a struck-by falling derrick or rotating tongs hazard: (a) On or about 01/25/06 at GWDC Drilling Rig#132 found near Bremond, TX malleable cast iron clips were used to secure the guylines, make-up and break-out tongs. The employer had also failed to install thimbles at the ends of the wire ropes securing the tongs where the eye was formed to prevent crushing of the wire rope. U-bolt wire rope clip U section was in contact with the live end of the rope. Among other methods, one feasible and acceptable abatement method to correct this hazard is to use forged wire rope clips and install them in accordance with the manufacturer's recommendations. Ref: ASME B30.9-2003 Section 9-2.3 para 2.3.1(a)(2) Malleable cast iron clips shall not be used to fabricate (wire rope) slings. 29CFR1926.251(c)(5)(i) When used for eye splices, the U-bolt shall be applied so that the "U" section is in contact with the dead end of the rope.
