Text For Citation: 01 Item/Group: 001 Hazard: CAUGHT
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a caught between hazard: a) On or about 09/25/05 an employee entered the work zone of an Automatic Fork Truck/ Crane and was fatally pinned between this equipment and a column. The employer failed to ensure that this equipment did not automatically perform/start an operation while an employee is in the work zone. Among other methods, one feasible and acceptable abatement method to correct this hazard is to interlock the access to the AFT/ Cranes work zone. Interlocking the access to the work zone would prevent automatic movement/ start-up of the AFT/ Crane while an employee in accessing the AFT/ Cranes work zone.
