Text For Citation: 01 Item/Group: 001 Hazard: FALLING

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards while being lifted on an elevated platform: (a) Springhill Plaza Site: Employees were exposed to fall hazards when lifted to the third story, approximately 30 feet, with a Rough Terrain Forklift Truck using a platform that extended more than 10 inches beyond the width of the truck on both sides. One feasible method to correct this hazard is to follow the requirements specified in ANSI Standard B56.6-1992, "Safety Standard for Rough Terrain Forklift Trucks," Paragraph 8.25.1(b), "Platforms for Elevating Personnel" which states, "Floor dimensions shall not exceed two times the load center distance listed on the rough terrain forklift truck nameplate, measured parallel to the longitudinal center plane of the truck, nor have a width greater than the overall width of the truck [measured across the load bearing tires] plus 10 inches (250 mm)] on either side. The platform should be fabricated so that it is only the width of the forklift plus 10 inches on each side.
