Text For Citation: 01 Item/Group: 001 Hazard: FALLING

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards while being lifted on an elevated platform: a.CRC building: Employees were exposed to fall hazards when lifted to the roof, approximately 21 feet, with a JCB 506C Loadall, using a metal platform that extended more than the overall width of the truck (measured across the load bearing tires) plus 10 inches on each side. The employees were exposed to fall hazards. Because the metal platform was longer than required, the Loadall could have tipped thus causing the metal platform to come off the forks of the lift and dropping the employees to the ground. The metal platform was not secured to the Loadall either. The metal platform was 192 inches long. The width of the Loadall, measured form the outside of both front tires was 92 inches. The ends of the metal platform were extending 50 inches on either side. The metal platform should have been no greater than 112 inches long. One feasible method to correct this hazard is to follow the requirements specified in ANSI Standard B56.6-1992, "Safety Standard for Rough Terrain Forklift Trucks", Paragraph 8.25.1(b), "Platforms for Elevating Personnel" which states: "Floor dimensions shall not exceed two times the load center distance listed on the rough terrain forklift truck nameplate, measured parallel to the longitudinal center plane of the truck, nor have a width greater than the overall width of the truck (measured across the load bearing tires) plus 10 inches on either side.
