Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of falling into the elevator shaft and/or being struck by the elevator car: a)On or about 2/24/05: Freight/passenger elevator provided for moving furniture between two floors was not provided with car and hoistway doors or gates with an interlock system at each floor. Among other methods, acceptable and feasible abatement methods to correct these hazards are to: 1.Install car doors or gates at the entrance of the elevator car with an electric contact that shall be positively opened by a lever or other device attached to and operated by the door or gate; and that shall be maintained in the open position by the action of gravity or by a restrained compression spring, or both, or by a positive mechanical means in accordance with ANSI/ASME Z17.3-1989, Safety Code for Existing Elevators and Escalators, Section 3.4.2. 2.Install hoistway doors or gates at landing openings which guard the full width of the opening and guard the height to a minimum of 6 feet above the landing in accordance with ANSI/ASME Z17.3-1989, Safety Code for Existing Elevators and Escalators, Section 2.6. 3.Install door interlocks on the hoistway doors. The interlock device shall lock the door in the closed position before the elevator can be operated and prevent the door from opening when the platform is not parked on that level in accordance with ANSI/ASME A17.3-1986 Safety Code for Existing Elevators and Escalators, Section 2.7.1. Abatement certification must be submitted for this item.
