Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to THE HAZARD OF BEING STRUCK-BY: a)Outside, Yard Area, on or about 1/27/05: A 1994 Komatsu model WA180 Front End Loader, sn.A75826 which was provided with a roll-over protective structure was not provided with a seat belt restraint system for the operator of the equipment. ABATEMENT NOTE: One means of abatement is to ensure through the manufacturer's operator/parts manual that all employee restraint systems that came with the equipment are maintained in an operable condition. b)Outside, Yard Area, on or about 1/27/05: A 1994 Komatsu model WA180 Front End Loader, sn.A75826 used by employees for clean-up, plowing and loading sand had an inoperative reverse signal alarm. ABATEMENT NOTE: One means of abatement is to ensure that all safety devices provided by the manufacturer per the operator/parts manual are maintained in an operable condition.
