Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a crushing hazard created by the sudden and unpredictable descent of vehicles elevated by automotive lifts: a)Shop Bays 1 and 2, on or about 07/27/2004: Two 10,000 pound hydraulic cylinder automotive lifts were in use with no safety or warning labels posted in a conspicuous location. b)Shop Bays 1 and 2, on or about 07/27/2004: Two 10,000 pound hydraulic cylinder automotive lifts were in use by employees and their training had not been documented. c)Shop Bays 1 and 2, on or about 07/27/2004: Two 10,000 pound hydraulic cylinder automotive lifts were in use by employees with no periodic inspection procedure established. Among other methods, feasible and acceptable methods to correct each of these hazards are to: 1.Ensure that safety and warning labels are posted to address general safety instructions, safety tips and potential hazards in accordance with ANSI/ALI ALOIM-2000, Section 4 para 4.2.2. 2.Ensure that operator training is documented by completing the Operator Training Log in the appendix of ANSI/ALI ALOIM-2000, Section 4 para 4.3. 3.Ensure that a periodic inspection is developed and maintained in accordance with ANSI/ALI ALOIM-2000, Section 5 para 5.1.
