Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being crushed by a raised vehicle. a)On or about 01/16/2009 - establishment, 172-70 Baisley Boulevard An employee was crushed by a 2008 Dodge Grand Caravan while attempting to release the cable that secured the spare tire located under the front center of the vehicle. The vehicle was raised on the jack. Among other methods, one feasible and acceptable abatement method to correct this hazard is to train employees on Jacking and Tire Changing procedures and warnings outlined in Chapter 6, What to do in Emergencies, of the Owner's Manual which states "Never get any part of your body under a vehicle on a jack. If you need to get under a raised vehicle, take it to a service center where it can be raised on a lift". WRITTEN ABATEMENT CERTIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19.
