Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by forklifts: (a)AFCO Steel LLC at 31455 County Road 39 1/2, Greeley, CO 80631: On or before 03/16/05, the employer did not provide back up alarms for the following forklifts: 1)Yale forklift, model YA3655, serial number A877D01615x; 2)Yale forklift, model GDP360EANPCV161, s/n A877D01592x; Both forklifts, which were originally equipped with working backup alarms, included a 9 foot blind spot behind the cab. The forklifts were used to transfer steel from the yard to the steel staging area, and also to move steel between workstations in the fabrication area. With no backup alarms and with a large blind spot behind the vehicles, fitting, welding, and yard employees were exposed to struck by and run over hazards by the forklifts. Abatement Note: Abatement certification is required for this item (See enclosed "Sample Abatement-Certification Letter"). Among other methods, one feasible and acceptable abatement method included following the manufacturer's original specifications, and ensuring the back-up alarm is in working order.
