Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which are free from recognized hazards that are causing or are likely to cause death or serious physical harm to his employees in that the employees are exposed to a struck-by hazard if the wire rope connection were to fail: (a) CDX-DART Drilling, at the Smith Rancho #2 wellsite, Moffat, County, CO: On or before December 27, 2004, the employer did not ensure that the specified spacing between clips on the wire rope that anchored the winch line on the drill floor was maintained, thereby exposing 10 or more employees to a struck-by hazard in the event the wire rope connection failed. The clips had no spacing between them. Among other methods, one feasible and acceptable method to correct this hazard is to place three inches between clamps. Abatement Note: Abatement certification and documentation are required for this item (See enclosed "Sample Abatement-Certification Letter").
