Text For Citation: 01 Item/Group: 001 Hazard: GUARDING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to amputation hazards: P/B Bering Star: On or about February 15, 2011 and times prior thereto, employees were exposed to amputation hazards while working on Header # 1, where the emergency stop button was not adjusted properly; exposing employees to amputation hazards as they adjusted the panel. Among other methods, a means of correcting this hazard would have been to have adjusted the emergency stop guard, preventing the employees from having to make adjustments while operating the header.
