Text For Citation: 01 Item/Group: 001 Hazard: ELECTRIC
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to electrical shock hazards: (a)American Seafoods Company (Processing area and Refrigeration Room) On or about March 5, 2008 employees were exposed to electrical shock hazards created by inadequate methods of securing LOTO tags to machinery, multiple personnel listed on a single LOTO tag, LOTO devices installed without the use of identifying tags and the failure of all affected personnel working on machinery to place their personal lock on the equipment. In accordance with American National Standard (ANSI) Z244.1-2003, one feasible method of abatement would be for the employer to ensure that all authorized personnel attach LOTO tags with adequate 50# rated attachments, that all authorized personnel place their personal locks on a multiple openings lock box, assign personal tags to employees for their own use and install identifying tags when using LOTO devices. .
