Text For Citation: 01 Item/Group: 001 Hazard: LIFTING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employee were exposed to fall hazards: a) On or about November 1, 2004, and times prior thereto in the Adams Golf Warehouse; an employee was elevated on a pallet attached to the forks of a reach truck so he could access material off shelves. The employee was transported both laterally and horizantally while standing on the pallet. The pallet on the reach truck did not constitute an appropriate work platform. FEASIBLE AND ACCEPTABLE ABATEMENT METHODS TO CORRECT THE HAZARDS INCLUDE, BUT ARE NOT LIMITED TO, THE FOLLOWING: 1. When a work platform is used on trucks designed and intended for handling materials, the requirements of American Society of Mechanical Engineers (ASME) B56.1-2000, Safety standards for Low Lift and High Lift Truck Section 4.17.2 and 4.17.3 should be met to protect personnel. 2. Provide a work platform that complies with the design requirement in ASME B56.1-2000 Section 7.36. 3. Securely attach work platform to lifting carriage or forks. 4. All travel controls should be in neutral and the parking brake set when personnel are elevated.
