A.R.S. 23-403.A.: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to his employees in that employees were loading and unloading storage racks that were not anchored to the floor, did not have a load rating posted and were not free from structural damage. (a) Storage Room and Flow line Room: The employer did not ensure that each footplate of the 1 5/8" x 3" storage rack columns was anchored to the floor. (b) Storage Room and Flow line Room: The employer did not post a legible printed plaque in an easily visible location stating the maximum load capacity per bay. (c) Storage Room and Flow line Room: The employer did not ensure that the storage racks were kept free from damage.
