Text For Citation: 01 Item/Group: 001 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falls from heights. (a) Ben Hill United Methodist Church site, fellowship hall, "A" quadrant roof, southwest side of building, On or about 5/10/05 - The employer failed to follow manufacturer's guidelines for installation of the 3/8" shoulder eyebolts used as a component of the guardrail system. The eyebolts were required to be firmly seated and flush against the concrete surface to prevent significant reduction of their rated capacity. Hazard of falls to a lower level. Among other methods, one feasible and acceptable abatement method to correct this hazard is: To follow and implement the Ken Forging, Inc., manufacturer's Eyebolt Information, Application/Installation section, Bullet 7, which requires that shoulder eyebolts be firmly seated and flush against the mating surface; otherwise, the rated capacity is reduced significantly. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED
