Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing injuries and/or death from falling trusses: Work Site: The employees, performing the roof truss erection were exposed to crushing injuries and/or death due to the following deficiencie: 1. Roof trusses were stored on uneven ground and exposed to the elements causing potential deformation. 2. Employees used 12d nails when 16d nails were required. 3. Temporary and permanent bracing were installed without site specific procedures. 4. Employees were inadequately trained in roof truss erection procedures. Consequently, on or about August 17, 2004, the entire roof truss structure underwent a catastrophic failure and collapsed. Among other methods, one feasible and acceptable abatement method to correct the hazard is to: 1. Utilize proper delivery and storage of wood trusses. 2. Utilize proper documented erection procedures. 3. Utilize trained wood truss erection workers. 4. Utilize skilled and experienced supervision. 5. Utilize correct nail size for temporary and permanent bracing attachment.
