Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
5(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970: the employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees. Section 5(a)(1) of the Occupational Safety and Health Act of 1970: the employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to point-of-operation crushing and amputation hazards without using safety blocks, slide locks, or other slide support systems. On January 8, 2004 employees were conducting die servicing and/or maintenance on dies while installed in hydraulic presses, company no. 406, and press no. 410, located in department 1119. One feasible and acceptable abatement method to correct this hazard is to require employees to use die safety blocks as referenced in ANSI B11.2-1995, Hydraulic Power Presses - Safety Requirements for Construction, Care, and Use, paragraph 9.2.6.
