Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of failure of wire rope or alloy chain due to improper rigging: (a)Processing area; wire rope cable for the hide puller was crushed. Hazard of the wire rope failing and snapping under load. (b)Loading Dock; alloy chain, used to pull cows out of the trailer, was hooked into the bucket of the bobcat. The hook throat was spread due to the pressure applied to the tip of the hook during pulling ("Tip Loading of the Hook"). Hazard of the alloy chain hook coming out of the bucket and snapping under pressure. (c)Loading Dock; the wire ropes, used to support the trailer tail gate, had rope clips installed backwards and a knot in one leg of the rope. Hazard of the wire ripe slipping out of the clips. AMONG OTHER METHODS, FEASIBLE AND ACCEPTABLE ABATEMENT METHODS TO CORRECT THESE HAZARDS ARE TO: (1)Replace wire rope on the hide puller. (2)Replace the bent hook on the alloy chain. Design an appropriate anchorage and/or attachment point on the bucket of the bobcat for the alloy chain hook to attach into. (3)Orientate the wire rope clips properly and install the proper length of wire rope so a knot is not needed to take up slack. (4)Establish an inspection program, with trained individuals, to perform periodic inspections of the wire ropes, alloy chains and related equipment.
