Text For Citation: 01 Item/Group: 001 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a fall hazard: (a)At 2000 McFarland Blvd, Tuscaloosa, AL - McDonalds Restaurant - On or about 10/29/03, an employee working from the ladder of the S47 Aerial Ladder Truck were exposed to the hazard of the ladder retracting and/or collapsing due to a defective cable. The 40' by 5/16" cable had not been replaced since 1999. Among others, one feasible and acceptable method to correct this hazard is to inspect and replace the cable as required by the Wilkie Manfacturer's Manual which states under "Lubrication & Inspection Specifications", page 7: Inspect all steel cable for any broken strands, excess rust or corrosion or damaged areas. Replace if any of these are found in cable. Install new cable as per J. Page 9 - "Inspection & Lubrication Record" -"J" - Install new cable every 24 months.
