Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the condition(s) listed below: Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: At the job-site, Trail Mountain Lease, Helen # 8, located 3 miles south of Sundown, TX, for the period of 02/23/04, and times prior thereto: Employees required to use the Geronimo escape device to evacuate the derrickman's working platform in the event of an emergency were exposed to hazards associated with falls from heights of approximately 90 feet and resultant bodily impact with objects located at ground level, created by the improper installation of the Geronimo escape device as follows: a) The installation of the cable and escape device did not provide adequate means of access for an employee to securely mount the device prior to leaving the derrickman's work platform. The device was not installed in accordance with the manufacturers operators manual. The device was not installed in accordance with Employers policies and procedures Employee Health, Safety, and Environmental Handbook, Third edition, 7/03, chapter 14.10, that the device shall be installed according to manufacturer's recommendations. b) The anchor point of the Geronimo escape device was not located at the proper distance from the derrick. The length of the cable did not meet the manufacturers requirements or the API Recommended Practice, 54, Third Edition, August 1999, paragraph 6.10.1. The device was not installed in accordance with Employers policies and procedures Employee Health, Safety, and Environmental Handbook, Third edition, 7/03, chapter 14.10, that the device shall be installed according to manufacturer's recommendations.
