Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the condition(s) listed below: a) The work site located in Lipscomb County, Texas, Service Drilling Southwest LLC Rig #12; on or about September 13, 2003, employees were exposed to a struck-by hazard created when the blocks hit the crown of the rig and fell to the rig floor during TOH operations. Among other methods, one reasonable method of hazard abatement would be to provide a safety device which would control the drawworks by eliminating the possibility of pulling into the crown and would stop the traveling block at a safe level automatically.00y0 L
