Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees from being struck by the load from a defective or deficient hoist breaking in the precast building: The hoists of the precast building were not inspected and several of them had defective parts, including but not limited to, a bent I-beam and a disengaged pulley arm power source assembly on hoist #28. Feasible and acceptable abatement method to correct the hazard includes, inspecting and maintaining all the hoists in accordance with ASME B30.16-2003, "Overhead Hoists (Underhung)" and the manufacturer's recommendations. Periodic inspections should be conducted on a semi-annual or quarterly basis, as required by ASME B30.16-2.1.1(b)(2) for heavy hoist service and severe hoist service respectively. Such inspections would indicate high exposure to wear, deterioration, and malfunction of the load chain, pulleys and hooks, which, if left unattended, could result in possible performance problems that could lead to serious injury if the load was to fall. The hoist was used to move loads of heavy concrete slabs and hoppers full of wet concrete.
