Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing injuries: (a) Batch Control Panel: On or about 10/16/04 employees were starting the conveyor system from a remote site and the conveyors were not equipped with audible alarms and delayed start-up mechanisms. (b) Conveyor belt system: On or about 10/16/04 employees were shoveling cullet onto the conveyor belts that were not equipped with emergency stop cables and switches. Among other methods, one feasible and acceptable means to correct these hazards is to comply with the requirements outlined in the American National Standards Institute ANSI/ASME B20.1 Safety Standards for Conveyors and related equipment.
