Text For Citation: 01 Item/Group: 001 Hazard: EXPLOSION
40.1-51.1(a) of the Code of Virginia: The employer, Coors Brewing Company, did not furnish a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to fire and explosion hazards while working on or around sludge storage tanks. Inspections and routine maintenance had not been performed according to manufacturers recommendations. Inspections and maintenance had not been performed on the Shand & Jurs Flame Arrester, Model 94306 and the Shand & Jurs Pressure/Vacuum Breather Valve, Model 94020 which were located on top of a 440,000 gallon sludge storage tank at the Waste Water Treatment Facility at Coors Brewing Company, Rt. 340, Elkton, VA. The flame arrester acts as a end of line fire barrier between flammable liquids stored in low pressure tanks and its emitted combustible vapors. It provides a positive flame stop for flammable liquids. One feasible and acceptable abatement method, among other methods, to correct this hazard would be to inspect the bank of the flame arrester at least once a month for the first year after installation and clean the tube bank whenever the inspection shows the need as recommended by the Instruction Manual for the Shand & Jurs Flame Arrester. These inspections and the frequency of cleaning will provide the basis to establish a predetermine maintenance program. The Instruction Manual for the Shand & Jurs Pressure/Vacuum Breather Valve recommends inspection and maintenance a minimum once every three months.
