Section 5(a)(1) of the Occupational Safety & Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause dealth or serious physical harm to employees, in that employees were exposed to serious bodily injuries from falls due to a lower boom collapse on a aerial lift. Location - 40 Mawney Street, Attleboro: On or about 05/20/04, The lower boom failed on the Holan 819 aerial lift while it was in an elevated position. Feasible abatement methods include but are notlimited to the following: 1. Implement a comprehensive safety program with written procedures for instructing aerial lift operators to perform daily inspections as recommended in the equipment operations and maintenance manual. 2. Ensure vehicle mounted aerial lifts have a complete parts examination annually, including the stability tests for inspection of weld cracks on upper and lower booms, as required by ANSI/SIA 92.2-2001. 3. Ensure the boom is secured properly before traveling, to prevent afflictive shock on welded joints.
