Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards associated with an inadequately secured and braced bundle of granite slabs in a container on a truck trailer.: 300 Oak Street, Unit 920, Pembroke, Massachusetts: On or about 10-24-03 a bundle of granite in a container on a trailer tipped over crushing and seriously injuring a worker offloading the bundles. Feasible and useful methods to correct this hazard, among other methods: 1) Inspect bundles and bracing prior to entering the trailer to ensure that bundles are straight and secure against tipping, sliding, or falling. 2) As bracing is removed, ensure that the remaining bundles are secure against tipping, sliding or falling. 3) During the unloading process, ensure the remaining slabs and bundles are adequately secured to prevent them from falling on the workers. 4) Request the bundles to be shipped on "A" frames for increased stability. 5) Develop written procedures to address safe offloading procedures and methods to secure loads against tipping, sliding, or falling.
