Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm: At the jobsite employees were allowed to conduct work operations to replace the down hole pump on a Bethlehem pumping unit, model #640D-37S-144, serial #S-107, using a work over rig #1263, model 396-205-C, serial #127790, without the horsehead clamp bolt installed per the operating and maintenance instructions. Employees were exposed to the type of hazards associated with fatal injuries due to being struck by the falling horsehead. Among others, one feasible and acceptable method of abatement would be to follow established company work rules and recognized industry practice by assuring the horsehead is tightened, locked and/or latched.d,
