Text For Citation: 01 Item/Group: 001 Hazard: UNAPEQUIP
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the following: a.The Case Uniloader Model 1840, Serial #JAF0326617 was not properly maintained nor were employees trained per the operator's manuel to safely operate the Case Uniloader, in that: a.The safety bar was not operating properly in that, when the bar was raised, the squeege descended. b.There were no bulbs in the head lamps. c.There were no bulbs in the tail lamps. d.There was no training program for operators of the front end loader. Among other methods, a feasible and acceptable method to correct this hazard would be to inspect the function of the safety bar to ensure it was operating properly, as prescribed in the Operator's Manual, implement a training program to familiarize employees with safe operating procedures, and to install bulbs in the head lamps and tail lamps.
