Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards: (a) On or about 12/09/03, Interlake metal racks, holding mattresses, bed frames, head boards and other boxed furniture, were not anchored to the floor per the manufacturer's installation instructions, thus exposing the employees the hazard of being struck or crushed by falling material or shelf systems. Possible means of abatement include: (1) To securely anchor the foot plate of every column on each pallet rack to the floor per the manufacturer's instructions. (2) To follow rack manufacturer recommendations regarding proper storage and use. (3) To instruct the employees of the approved rack load capacities, enabling them to verify the maximum amount of material that could be safely stored.
