Section 5(a)(1) of the Occupational Safety and Health Act: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck or crushed by paper machines and equipment in that: a.)On or about August 25, 2003, and times prior thereto, the employer did not provide adequate training for employees responsible for the daily operation, servicing and maintenance of the paper machines. Conditions noted at the time of this inspection which contributed to this hazard included: 1)The shift supervisor did not verify and certify that their employees were adequately trained for the jobs they were performing. 2)The training record did not include the date training occurred and the duration of training as required by the corporate policy 553-SOP-002, Revision Date of 06-04-99. 3)The employer required employees to fill a position although they have not been adequately trained and certified for that position. Among other methods, one feasible and acceptable abatement method to correct this hazard is to ensure that employees are provided with adequate training so they can perform their job duties in a safe manner. In addition, the employer must amend, update and enforce the employer's corporate policy "553-SOP-002, #1 and #2 Paper Machines Employee Training".
