Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being crushed or struck by the forklift in the event of truck tipover: a)South side of the West sludge holding tank, on or about 4/28/04: An Allis- Chalmers forklift, s/n 1745, used by employees was not provided with a seat belt. ABATEMENT NOTE: One means of abatement is to reinstall the seat belts which were on the forklift and insure that all employees that operate this forklift wear the seat belts at all times during operation.
