Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of getting caught or struck by conveyors: a)Throughout the Sand System Conveyor, On or about 2/19/04: There were no start up alarms for the system. b)Sand Return Conveyor #3, On or about 2/19/04: The emergency stop was not operational. Among other methods, feasible methods of abatement are such as but not limited to: 1) Ensure that start up alarms are used or that the system is 100 percent guarded as per ANSI B20.1 2000, section 5.11.2., and 2) Ensure that safety devices are maintained in a serviceable condition, including emergency stop devices, as per ANSI B20.1 1996, section 5.9.1.4. Abatement documentation must be submitted for this item.
