Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of falling into the elevator shaft and/or being struck by the elevator car: a)Main Building, on or about 9/17/03: A freight/passenger elevator provided for moving stock and parts between three floors was not provided with doors so interlocked at each floor as to prevent their opening when the platform is not located at that floor and the elevator car did not have interlocked doors to fully enclose the operator. Among other methods, acceptable and feasible abatement methods to correct these hazards are to: 1.Install door interlocks on the hoistway doors. The interlocking devices shall lock the door in the closed position before the elevator can be operated and prevent the door from opening when the platform is not parked on that level in accordance with ANSI/ASME A17.3-1986 Safety Code for Elevators and Escalators, Section 2.7.1 2.Install car door or a gate at the entrance of the elevator car positively opened by a lever or other device attached to and operated by the door or gate in accordance with ANSI/ASME A17.3-1986 Safety Code for Elevators and Escalators, Section 3.4.2. Abatement certification must be submitted for this item.
