Text For Citation: 01 Item/Group: 001 Hazard: LIFTING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by the load: a)Equipment and Mixing Rooms, on or about 8/15/03: The following hoist support structures had unknown load capacity ratings: 1) 1-Ton hoist support structure in the Equipment Room. 2) 1/2-Ton CM hoist support structure in the Equipment Room. 3) CM Loadstar hoist support structure in the Mixing Room. Among other methods, one feasible method of abatement is to inspect the support structures for the hoists as per ASME B30.16-1993, to ensure the structure is designed to withstand the loads and forces imposed by the hoist for the rated load and mark the structure accordingly. b)Equipment Room, on or about 8/15/03: An under-the-hook lifting device attached to a 1-ton hoist had an unknown load capacity rating. Among other methods, one feasible method of abatement is to determine the load capacity rating for the under-the-hook lifting device and mark it accordingly as per ASME B30.20-1993. c)Mixing Room, on or about 8/15/03: CM Loadstar hoist had a missing safety latch from the hook. Among other methods, one feasible method of abatement is to install a safety latch as per ASME B30.16-1993.
