Text For Citation: 01 Item/Group: 001 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falling into the elevator shaft from a height of up to 15 feet. a) On or about 07/31/2003 at the shipping area freight elevator: The elevator hoistway was not provided with doors so interlocked at each floor as to prevent their opening when the platform is not located at that floor. Among other methods one acceptable and feasible abatement method to correct this hazard is to: 1) Conduct inspections of the elevator to assure that the doors and interlocking devices are in safe operating condition. 2) Follow the requirements of ANSI/ASME A17.3-1986 sec. 2.7.1(b) which requires that hoistway doors for freight elevators shall be equipped with door interlocks. The interlocking devices shall lock the door in the closed position before the elevator can be operated and prevent the door from opening when the platform is not parked on that level.
