Text For Citation: 01 Item/Group: 001 Hazard: LIFTING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by the load should the overhead hoist and components fail: a)Foundry, Core Room, by the Palmer Mixer, on or about 3/18/03: Upper limit device on an Ingersoll Rand 1/2 ton overhead hoist was not functioning. b)Foundry, Ladle Repair, on or about 3/18/03: A Monotractor #99 three ton overhead hoist had a broken safety latch at the lifting hook. c)Foundry, Inliner, Drill area, on or about 3/19/03: An Ingersoll Rand two ton overhead hoist #O2CH03 had several kinks in the wire rope, the upper limit switch was not functioning and there was no safety latch for the hook where the hoist attaches to the trolley and on another Ingersoll Rand two ton overhead hoist #02CH02, the upper limit switch was not functioning. d)Paint Dip Room, on or about 3/19/03: An Ingersoll Rand 1/2 ton overhead hoist had a broken safety latch where the hoist attaches to the trolley. e)Foundry, Core Room, on or about 4/1/03: A P & H two ton overhead hoist near the U360-1 and U180 core machines had kinking in the wire ropes and the upper limit device was not functioning. Among other methods, several feasible and acceptable abatement methods to correct these hazards are to: 1.Repair the upper limit device in accordance with ANSI B30.16-1993 Overhead Hoists, Chapter 16-1.2.14 2.Inspect the upper limit device at the beginning of each shift to ensure that it is functioning in accordance with ANSI B30.16-1993 Overhead Hoists, Section 16-2.1.2. 3.Install new safety latches on the hooks in accordance with ANSI B30.16-1993 Section 16-1.2.9. 4.Replace wire rope in accordance with ANSI B30.16-1993 Overhead Hoists, Section 16-2.4.2.
