Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a crushing hazard. a) Maintenance Shop - A maintenance employee was fatally injured when the pallet jack he was servicing fell from the hydraulic lift on which it was elevated. The employee had not been trained in the proper method of securing the pallet jack on the lift. He had also not been trained in the proper sequence of steps to take to service the pallet jack. On or about 02/04/03. One feasible and acceptable abatement method is to develop a written Standard Operating Procedure for this activity. The SOP should include a safe method for supporting the pallet jack on an overhead hydraulic lift while pins are removed to permit service operations to take place.
