Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The the employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing injuries while operating a vehicle with a disabled alarm: a) On or about March 6, 2004, U.S. Waste Industries, Inc., did not ensure that employees were protected from injury, in that the tailgate alarm had been disabled on truck (#408) and on several other trash trucks. Among other methods, one feasible and acceptable abatement method to correct this hazardous condition would be to develop an inspection criteria in accordance with ANSI Z245.1-1992, Section 6.1(k) to ensure proper operation of safety equipment and protective devices. Abatement Note: Abatement certification and documentation is required for this item (see enclosed "Sample Abatement-Certification Letter").
