A.R.S. 23-403.A.: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to his employees in that employees were not given information and training in the recognition and avoidance of hazards associated with the operation of press brakes. ANSI B11.3 titled Safety Requirements for the Construction, Care, and Use of Power press brakes mandates that employees be trained in the safe method of operation prior to use: (a) 602 South 65th Avenue Phoenix, AZ 85043: The employer did not instruct each press brake operator in the safe operation of such equipment.
