Section 5(A)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or were likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being pulled into the rotating parts of a pipe threading machine: On or about 02/28/03, an employee was using a RIGID 300 electric pipe threader which was not provided with working foot pedal control switch as required by American National Standards Institute B208.1-1982, Section 4.4. Among other methods, one feasible and acceptable method of abatement is: Provide the pipe threader with a constant foot threader control switch.
