Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that they were exposed to falling/struck by hazard: a) On site, rear building - Employees gained access to the roof area by being lifted by a CAT rough terrain industrial truck with a metal platform held in place on the forks by putting a wrench through a hole made in one of the one forks to hold the platform on the truck. The platform was missing a midrail, employees were climbing onto the top rail to gain access and egress, and the width of the platform exceed the overall width of the truck on or about 3/4/07. One means of feasible abatement, among others, is to follow the manufacturers requirements as set forth in American National Standards Institute (ANSI) Standard B56.6-1993, Safety Standard for Rough Terrain Forklift Trucks, Section 5.15 and Section 8.25 of this standard for elevating personnel and the design requirements of the platform.nts
