Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of fires when the hot rollers (heated elements) on Garnett Line #1 were not set to the correct line speed, which caused the garnett to wrap around the over 900 degree F roller: On February 3, 2003 employees were running Line #1 through the hot rollers when the fiber insulation web (garnett) generated an area of charred fibers. One feasible and acceptable method of abatement to correct the hazard of fires is to perform a complete and thorough hazard analysis to identify, evaluate and control all the hazards involved with the web operation and the heated rollers. Another acceptable method of abatement is to install four sets of rollers to reduce the potential of the garnett wrapping around the hot rollers.
