Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck or crushed by overhead loads: (a) Location - Load bar (lowerator) #23: On or about 06/20/2003, employees were exposed to the hazard of being struck or crushed by overhead loads while utilizing hoisting equipment. Among other feasible methods of abatement, one acceptable method to correct the hazard is: 1) Develop and implement a more reliable locking mechanism on the lowerator bar. 2) Train employees in the safe operating procedures for the trolleying of overhead loads. 3) Develop and implement a standard maintenance and inspection procedure of all overhead hoisting equipment.
